Based on your chosen family’s health and wellness assessment that you have just completed, answer the following:

• What is your overall recommended comprehensive health screening appraisal for the individuals in your chosen family?
• What health risks have you assessed based on genetics, environment, health behaviors, and social factors?

Part 2 individual project

Based on the health and wellness assessment you conducted on your chosen family, complete the following:

1. Construct a health and wellness plan of care to improve the health status of the family (child, adult, and older adult).
2. Research available resources in the community that may assist the family with achieving optimal wellness. Include them in your plan (along with contact info for the resources).
3. At least 1 area of the plan must address a Healthy People 2020 initiative.
4. The plan must include evidence of an interprofessional collaboration in the community to help the family achieve health and wellness.
5. The plan must provide evidence of consideration of individuals with diverse life experiences, perspectives, and backgrounds.
6. Identify strategies for delivery of the health and wellness plan in the virtual environment.

You will be required to deliver the health and wellness plan to the family in Week 4. Faculty must approve the health and wellness plan prior to delivery.

The health and wellness plan must be a minimum of 10 pages (excluding the references). The plan should be written at a 5th-grade level, and bullet points are best (or short paragraphs). Pictures may be used, but pictures may not take up more than 2 of the 10 pages. The plan must address health and wellness for the entire family (child, adult, and older adult) and will be a total of 10 pages. The health and wellness plan is expected to be professional and appropriate for dissemination to an external body.

Part 3

You are counseling a fictional individual on health promotion and risk reduction. This individual is generally healthy, but there is always an opportunity to educate. The individual’s spouse and two children are in the room as well. Select 2 health promotion opportunities to discuss with this individual and his or her family, and then answer the following:

• Why is health promotion essential for the family?
• How would you best educate the individual and the family to promote healthy living, specifically with the topics you have selected?
• How and when would you follow up with this family?

Part 4

Provide a 2-page executive summary of your health and wellness plan. Include a third page that serves as an evaluation of the delivery of the health and wellness plan to the family, including how the plan was received by the family and how likely the family is to need a follow-up health and wellness intervention.

Support your paper with at least 2 scholarly sources, excluding your text.
